What is a Community Health Center / Federally Qualified Health Center?

A community health center is a non-profit provider of high quality, cost-effective, preventative and primary health care to all populations, regardless of their insurance status.  Since 1965, community health centers have provided access to health care for millions of people in underserved urban and rural communities in all 50 states.  Health centers are designed on a comprehensive service delivery model to assist those most impacted by health access barriers, including low income, uninsured, those with limited English-speaking proficiency, migrant and season farmworkers, those experiencing homelessness, as well as those who live in public housing.
Community health centers are community-based and governed by a local board that is represented by a minimum of 51% patient consumers, ensuring that health center services are fit to their unique needs. Community health centers focus on wellness, disease prevention, and early intervention, thus reducing the overload in hospital emergency rooms, and saving the nation billions of dollars annually in health care expenses.  to ensure income is not a barrier to care, patients may receive a discount on their healthcare services, according to their income.
Today, over 1,200 health centers provide cost-efficient care to over 20 million individuals nationwide.

What is a Patient Centered Medical Home?

A Kaniksu patient centered medical home (PCMH) is not a building or a place, but a team of Kaniksu health professionals who work together to provide a central point for coordinating care to help you become as healthy as possible. 
Led by your primary care provider, your Kaniksu PCMH team will:
  • Care for you as a whole
  • Provide you with better, more personalized care
  • Guide you through the complex healthcare system and track ALL your healthcare reports and medications
  • Offer you better access to care through longer hours and more services
  • Partner with you to help you live healthier everyday
