- Copy of your Resident Alien Card (front and back)
- Copy of a residency document issued by the Department of Homeland Security
- Copy of paycheck stubs for the most recent 30 days
- Work Verification form signed by your employer
- Copy of your most recent tax forms, if self-employed (include all schedule forms)
- Completed Self-Employment Verification form
- Copy of statements or documents for income not earned from employment
(veterans benefits, out of state unemployment, retirement income, workers compensation) - Statement or printout from Child Support agency
- Signed statement from the person receiving child support payments
- Copy of bank statements for the most recent 30 days
- Printouts from your bank
(should include your name, account number, and balance) - Copy of statements or other documents that prove the amount you owe on any vehicles
- Printouts or statements that show current value of stocks, bonds, certificates of deposit, trust accounts, etc.
Child support expenses
- Copy of paycheck stubs that show child support payments withheld
- Copy of court documents showing the amount you are legally required to pay
- Signed statement from the person receiving the child support payments
- Copy of a statement showing the past 3 months income from the Child Support agency, if not paid through Idaho
- Dependent Care Charges form completed by the child care provider
- Statement from your child care provider

THE AFFORDABLE CARE ACT
In 2010, a comprehensive health reform was signed into law known as the Patient Protection and Affordable Care Act (ACA), commonly called ObamaCare. The law makes it possible for more families to have access preventive, affordable health care. If you have any questions about the Affordable Care Act, or need assistance enrolling in the Health Insurance Marketplace, contact us at (208) 263-7101 and we’ll have our Outreach & Enrollment Specialist contact you.
After the Healthcare.gov Marketplace enrollment period ends on January 31. You can still enroll in a Marketplace health plan only if you qualify for a special enrollment period. You can apply for Medicaid any time.
The following people may be eligible to enroll in health insurance through the federal marketplace before the next open enrollment if they qualify for the special enrollment period.

The Children’s Health Insurance Program, or CHIP, is a low-cost health coverage program for children under age 19 whose family income is too high to qualify for Medicaid. There may be a small monthly premium ($10 to $15, depending on income) for each child enrolled in CHIP.

• Take advantage of free preventive benefits, cancer screenings, and yearly “Wellness” visits.
• Most Medicare Prescription Drug Plans have a coverage gap (also called the "donut hole"). You can save money if you’re in the “donut hole” with big discounts on brand-name prescription drugs. Each year, there are new health plan and prescription drug coverage choices. You should review your current health and prescription drug coverage each fall, and make sure your plan’s still right for you.

- Are no longer eligible for employer-sponsored coverage.
- Lost eligibility for Medicaid.
- Recently married or divorced.
- Have had or adopted a child.
- Tried to enroll in the exchange but didn’t qualify for a tax subsidy because income was too low, but now have a higher income.
- Recently moved to Idaho or to a new county in the state.
- Turned 26 and lost coverage under a parent’s plan.
- Had a change in immigration status and are now lawfully present.
- Recently got out of jail.
Be Prepared
Get your required information prepared and find answers to your coverage questions and figure out what options you need. It’s good to be prepared, and now is the time to get started.
- Gather information regarding your household income. To help determine health savings you’re eligible for, the Marketplace will need income information from your W-2, current pay stubs or your tax return.
- Starting setting a budget to spend on your premiums each month. You will need to determine which health plan meets your families health needs, but also meets your budget requirements.
- Find out if your employer plans to offer health insurance. You will be able to stay on your employers plan, but will enjoy some of the benefits such as no denials of pre-exisisting conditions or coverage dropping when you’re sick.
- Begin the application process at the Marketplace.

- Adults with a child under the age of 19
- 65 or older
- Individuals who are blind or disabled according to Social Security Administration criteria.
If you fit one of the categories above, you must also meet the following eligibility criteria:
- You must be a citizen or legal immigrant, and
- You must be a resident of the State of Idaho, and
- Your household income must be less than the program income limits for your household size, and
- Your resources must not exceed the program resource limits.
